Supervised Diets
In my case, I did not really have any doctor supervised diets. I did, however, provide in my letter a history of my weight and diet loss/gains. I literally went back to 2nd grade, and gave almost a year for year account. I listed every diet I'd ever tried, when, how much I lost, how quickly I gained it back - I even charted the weight out by year for them (I'm a nerd! LOL!). It worked for me, however, I had close to six months the previous year when I had regular visits trying to get my blood pressure under control. Even though my chart at the office didn't say anything about a weight loss attempt, I was there and weighing in and used that to correspond I guess "proof of weight" along with my harrowing tale. The surgeon wanted to see it, but ultimately, it was a requirement for the insurance company.

In my case, the insurance is requesting this. I've been on a bunch of diets, but now, because they were never monitored by a physician, I have to pay out of pocket for the nutritionist that is required by Aetna, who, by the way, refuse to pay for because they denied my surgery. Just make sure that you get everything charted when you see your PCP, cause if you don't that could be the one thing that gets you denied.
