Question?? TeamCare Central States, Southeast and Southwest areas Healthcare

Has anyone been approved by TeamCare Central States, Southeast and Southwest areas Health and Welfare fund PHCS/ Private Healthcare Systems. OH MY GOSH can you imagine how difficult their forms will be it their name is that long!! I am so afraid of being denied and setting myslf up for failure.
