Care Credit form for weight wise?

 I need a care credit form to fill out and fax in to Cheryl at WW this weekend/Monday morning; she was supposed to email me the form today and that didn't happen... Does anyone out there have a copy of the form they use? If so, could you email that to me? I have to pay the self pay amount on Tuesday and it's going to take at least Monday to find out if they even approve me for the full amount. I'm nearly bonkers at this point : )

U should be able to go to the website and sign up for credit. it doesnt take too long and they will tell you what your limit is and send your card in the mail in a couple of weeks. There is also something that you can print off for verification and take it to your provider. Care credit is good...I use it for my cats appts.

I applied online and got instant approval.  Shouldn't be an issue.
