HELP!!! IM FREAKIN' OUT CAUSE MY 1ST CONSULT GOT MOVED UP!!

If anyone has last min. suggestions on what I'll need before I go to my first consult, I would appreciate it. I have my five year weight history, and my packet filled out. ( I'm going through St Vincent Charity Hospital) I dont want to go to this and find out that I needed one piece of paper and I forgot it at home. There should be a checklist somewhere.(sorry thining out loud)

I will dig my papers out this evening and see if I can think of anything you might need. When I went I just took anything and everything. That way there was nothing else they could possibly need. Just take some deep relaxing breaths and thank God your consult got moved up.. Yeee Haaaa!!! Hugs Laura

I went through St. V's and don't remember taking anything other than my insurance card with me. Who is your appt with?

I took everything I had: I took my five year weight history (from three different doctors), test results from my doctor, a personal letter to my insurance company w/pictures, and copies of proof of previous weight loss attempts. After my consult, I still had to get my psych eval and then they required me to get a sleep study. It was a pretty smooth process, it's they waiting game that sucks. They have a check list at St. V's and your case manager will go over it with you and let you know what all you need to get to them. Good Luck! I am having surgery on 7/28 at St. V's. Stacey

test results?? from what or i guess i should say what kind? How do you get proof of weight loss attempts? I went to l.a. weight loss for at least 6 mo.?
