Hi my name is rich, i've posted on here before about qualchoice but i have a few more questions.i know that they want 5 yrs. of weights, but what if u only have like 3 yrs instead of the 5? i dont have 2003,2004. and ive never did any of the commerical weight loss things,ex. weigh****chers, la weight loss , etc...i have a consult with dr. hallawell at UH on july 18th. has anyone had these problems? i know i've seen people post about sending pictures if they dont have the documented weights but i dont know if qualchoice will accept them. please any help will be appreciated. rich w

Rich, I am going through UH also. I have Medical Mutual and they required a 5 year weight history. My PCP gave them 5 years of weights dating from 1992......I was missing 2000, 2001, 2002, 2003....so basically the same boat as you are in. I did send MM pictures of me during those years. I have grandkids and I always shied away from the camera....how my daughters got pictures of me with the kids is beyond me...but they did. It was really a great way of documentation! I was holding my granddaughter in each picture of the years missing. I sent them and told them I would be happy to give them a copy of her birth cert. they didn't ask so I didn't. They then asked me for a photo ID....driver's lisence or passport.....so I took my ID to Kinko's....taped my insurance card so it was covering my weight....and then added my Sam's card too! Sent it in to them and was approved in less than 5 days! Nicole will help you through the insurance process. When it comes to diet attempts list things you did at home....cutting out carbs......low fat....low calorie....buy an Atkin's book....how can they prove that you didn't try it? I didn't have much to offer with this but Nicole sent it through without any questions from MM. She is really great! Dr. Jasper will be doing my surgery on Wednesday! Good luck! And go to as many pre-op support group meetings that you can! You will learn from each one! Sue

Hi Rich, I also have qualchoice and am 7days post op. I had to retain the same information and I found all they wanted was a documented number of your weight not five years of diet. I went my family physcian and they gave me the info. with pleasure. Try and think of any Dr. apt you may have gone for emergency, blood work, xrays, msc... They are very easy to deal with I had my aproval in 24 hrs. Good Luck Toni L in Ohio

toni is just your companies policy for qualchoice changing or is all of qualchoice changing? there isnt anything in my paper work that says anything about changing the policy. dont they have to let you know about the changes? about the original post i dont have the 5 yrs history since i didnt see a dr in 2002,2003.
