hi all my fellow ohioians i was wondering if someone who went threw ohio state for sx can tell me the steps that we need to go, i talked to shannon this morning but i was in a sleep daze so if someone could tell me i would apprecaite it thanks again have a happy valentines day i have a date with the slot machine lol
I am going through OSU.

It explains their processes. My insurance required me to do a 6 month supervised diet. I have about 3 weeks left of the diet, then my information will be sent to OSU so they can send it off for approval.

I'm going thru OSU, they require depending on your insurance, either a 3 or 6 month class at the OSU Center of Wellness and Prevention, the 3 month class costs $300, I had to pay out of pocket, but depends on your insurance company, I would suggest calling them direct and asking them about how long you would have to attend class and insurance coverage and out of pocket expenses. Their number directly is 614-293-2800. Call them as soon as you find out your insurance requirements and get an orientation done so you can get the class started, They put you on a 1200, 1400, or 1600 cal diet and you have to attend class once a week for the 3-6 months. You could always wait till you send in your application to OSU, they will then tell you if need the class, or call Shannon yourself and ask her if you should start the classes
When your done with the living well classes they send the papers to the bariatric surgeons office, they then set you up with a dietian~Kris Fout and PSYCH evalw/Dr Kramer, I have Aetna and Dr. Kramer don't take Aetna so now I'm told I have to pay $415 Feb 24th to see him, and they won't let me see another PSYCH doctor, its him or go to another Bariatric Surgeon, that is what I was told today. The number directly is 614-293-9463 it is to Mary Jo Lee the insurance coordiator for Dr. Kramer, just ask if they take your insurance.
Make sure when you go to Living Well class you go to your doctor once a month just for weigh in and have him say he is monitoring you on your 1200, 1400, or 1600 cal diet, which ever one you chose.
Now if you have had 6 months of your doctor monitoring you each month, doctumentation of going to a Dietian for 6 months and seeing a Behavior Specialist then you don't have to go thru the Living Well class, but most of us don't. Like I said please call your insurance company and see what their requirements are. Luckly with Aetna they will let you do only 3 months of the living well classes, but I know some that have to do the whole 6 months. You will meet alot of really nice people there! I actually met my best friend PAULA there! SO it is worth it.
But OSU's requirements are To see the dietian and PSYCH doctor I mentioned above and it don't matter if your insurance requires to see them or not or even if your insurance overs them or not.
I now have to see the dietian and psych doctor the 24th and set up an appointment with my family doctor to give him a paper they gave me. It has everything they need faxed to them, referral, 5 year weight history, Documented diet history, Blood tests within the last 3 months( I have the list of tests needed if you want to have them done) and a copy of a history and physical exam down in the past year or recent office notes detailing andy co-morbid conditions. adn Any cardi-pulmonary testing done including ECG or EKG's. I would also recommend getting on your insurances website if they have one and finding what you need to do in order for the surgery and printing that off and taking that to your PCP doctor too!! But make sure you call them and see if its covered under YOUR plan! Some plans don't cover the surgery!!
I tell you what I wish I knew all of this before I started going to OSU to just give me a heads up to what to expect!
