diet history

After 2 years, I finally have an insurance that will cover surgery. I have to fill out a diet history. Has anyone else done this? I have never had a medically supervised diet. Just Jenny Craig, cabbage diet, things like that. Also, has anyone had Dr. Fallang for their surgery? Thanks, Julie [email protected]

Hello Julie, I wrote down EVERYTHING I had tried. You can see a copy of my diet history letter on my profile. I went to Dr.Fallang's WLS info session but then found out he was out of network for me. I then went to Dr's Maguire, Hadley, and Anderson. They are in Kettering Hospital. Good Luck, Robin W

Julie, Check out Dr. Trace Curry at Deaconess Hospital in Clifton. He is fantastic! He answers questions here all the time so ask away of him. PJ

Julie: I didn't have to put down any "medically supervised" diets. I just tried to remember what I had done and about when. In most cases, I had to fudge the dates because who can remember 5 years back what month you tried Atkins? There is also no way to prove you have done any specific diet during any specific timeframe.
