Med Mutual Ohio Med PPO
Every MMO policy is DIFFERENT. Call MMO and ask what YOUR POLICY states. Some have quit covering WLS, others have put a $10,000 cap on it, others have made NO changes.
I called MMO and ask what they needed to be approved. I provided all they wanted and was approved in about 15 days.
You can go online and see what doctors are in network at mmo.com, then go to find a doctor section.

My insuance is through the state with Medical Mutual Ohio Med PPO. I just got approved for WLS. It took only 9 business days for the approval. I am going through Barix which is out of network but I know OSU is in the network, also Springfield and Mt. Carmel hospitals I think. Good luck in the process.

I wanted to go through Barix too, but was told by MMOH that Barix was out of Network. My deductible was then upped to $300 and then was told that they would only cover 60% of the allowable from Barix. 60% of 20K leaves me with nearly $8000 out of pocket. So that was a bummer.
I did call the Bariatric Program at Mount Carmel and awaiting information from them.
I would have preferred Barix, but nearly $8500 Out of Pocket was pretty steep.

My husband works for the state and I had my WLS on 3/29 in Cincinnati at The Christ Hospital. It took months for approval because the ins co said they had not received my 5 year weight history from my PCP. Long story short - I had to do a lot of calling but finally got approved. Make sure you know what they expect you to send in for the review and make a copy of everything you send them.
They covered quite a bit and my out of pocket expenses were under $1000. We have Med Mutual Super Med Plus
