I am jumping through hoops with UHC and relaying info to Mid-Ohio Surgical (Mt Carmel in Columbus). Mid Ohio had a requirement supposedly that I need to do a Dr. Supervised diet for 6 months before scheduling of surgery. The UHC customer service have said that it is not listed as a requirement. I called Mid_Ohio and could only get answering machine today, but has anyone else been through this with UHC or Mid Ohio(Mt Carmel)? Thank you. Matt
P.S. I am a teacher and would love to have this done before going back at the end of August.
Hi Matt, I have UHC and I needed no proof of previous dieting. The Barix Center submitted my claim. They asked for several tests to prove health issues that were related to my obesity. My approval was very quick. I hope that this helps. Best of luck. I use to teach. All of my sons were summer babies. Schools hate teachers to miss days. Keep posting and let us know how things are.

If you have comobilities get all medical records and mail them to ur state Insurance board to help you fight it.
If you do the diet like I did with Aetna get urself a pocket tape recorder, it make it easier to keep documentations of what you have eaten thru out the day.

I didn't have any problems w/ UHC.
They only requested documented records from the Dr. of my weight and any conditions that were cause/aggravated by it.
I am having the lapband, which I understand is hard to get approved for.
I had approval w/in 2 weeks.
I am going through OSU which requires a 4 week class, psych eval and physical.
Time from getting paperwor****il surgery date will be approx. 7 mos.
