Help!!!!!

I need to throw a question out to all of you around the Columbus, Lancaster, and Logan area. Does anyone have any sugestions on a PCP that will recomend WLS around that area? I have an appointment on July 1st with one but I would like to get some other options if possible and I am fairly new to the area. Also what is the letter supposed to contain when they do write it so that I make sure that nothing is excluded because my insurance will not pay for the surgery after Jan 1, 05.
