Anthem PPO Insurance?

Has any one used or dealt w/ Anthem Blue Access PPO of Ohio ?

Hi John, Anthem is a tricky one. So far in my experience it is on an employer by employer basis. It is (in my opinion) easy to get an approval from Anthem once you have the data they require...psych eval, Dietitian eval, report from surgeon and the Physican Verification Form that Anthem provides. Once all completed someone in the surgeon's team can fax it to Anthem and get a response with in 48 hours. BUT many of Anthem's policies have plan exclusions--depending on the employer. This is not as easy with all of the BC/BC plans--just Anthem in OH. Call Anthem's customer service line and they will tell you if WLS is a "plan exclusion" or not. Sometimes they say it must be medically necessary--which means they can approve it. Hope it helps!! Good Luck!!! Cathy
