Amy, Try the following Psychologist: Wiliam Friday 5340 E. Main Suite 205 Columbus, OH 614-560-8220 The receptionist gave me the following codes for the insurance company: 96100 90801 90806 There is also a Douglas Kramer, but I don't have an address or phone #. Hope this helps Ginny
