have completed all the necessary paperwork have hit a snag i have mmoh super med plus doctor moved her office unfortunately they cannot find my old chart which is preventing me from getting my five year weight history which can be a major problem in getting my approval if any one has had this problem and got approval any info will be appreciated

Debra, sorry to hear that happened to you, if I were you I'd be really upset at the doctors office. I would tell the doctors office it's there responsibility to help you with the insurance company because they lost your files. Don't let them get away with just loosing your information. Good luck.

I concure with Kimberly. Stress with your doctor's office that you have hit a stumbling block and because they lost/missplaced the files you would like their help. If you have gone to the same doctor for those five years I don't see why they can't write a letter explaining the lost file and letting them know you were under their care. The doctor should remember at least general things about your care. Good Luck and let us know how this gets resolved.
