Did anyone need to have there psych eval completed prior to Dr. Martin's office submitting the pre-authorization to the insurance. I have submitted all paperwork to Dr. Martin's office and was told that I need to have the psych exam before they would submit it to UHC. UHC does not require a psych exam - Dr. MArtin does. Nicole

I have been learning about WLS also and have found that most doctors want a psych evaluation also. My insurance wanted one also so it will be covered. The only thing I had to pay up front so far was a $50 write up fee. Just call your insurance and see who's on the list, compaire that with who you doctor recommends. Good Luck, Robin W

I have UHC-PPO. I just called the Mental Health number on my insurance card for authorization, which I received over the phone. Then I set up the appointment for the psych evaluation, and gave the doctor the authorization numbers I got from the insurance company. I was surprised how easy it was! I went to Dr. Wilbert (office by Dayton Mall), because he is a UHC provider, and I could get an appointment for the following week. He faxed his report to my bariatric surgeon's office the next day. My cost was just the $15 copay, but it MUST be preauthorized. Dr. Wilbert's website is www.fattitudes.com Hope this helps. Nancy
