has anyone had surgery on medicaid at mount carmel or ohio state medicial.

No, but medicaid/caresource has pre-approved me. They don't seem to have an issue with bariatric surgery at all, so if you find a reputable place that you feel comfortable at, you shouldn't have a problem.
