Molina (medicaid) insurance & Dr. Spier

I finish my 6 months required supervised weightloss with my family doctor on May 5th and have an appt with Dr. Spier on May 9th.  I asked Jan from the office about the list of requirements they need, and she says that she has an entire sheet of the rquirements to give me on May 9th.

Was curious is anyone has had or is having surgery with Dr. Spier.  Did/do you have Molina? What was the list of requirements??? I know the appt isnt too far off but Id love to get a heads up of what Im walking into!

i also have molina and iam just getting stared with the process .iam trying to find out if they are going to pay for it.isee it been a while sence you post on here iam wondering have you had your surgery yet.i hope you have andi hope you are doing great.
