Individual/Private Insurance

 I was wondering anyone that has individual/private insurance and was approved for surgery, what insurance you have? I was looking into getting private, possibly anthem to get the surgery. I know anthem is a big one that covers alot of bariatric surgeries. My issue is how do I know which plan to choose without asking them upfront. I don't want to call and say I need insurance to cover bypass which policy should I have. I'm afraid they will either deny me the insurance or give me a super expensive quote. Any ideas on private insurance would be greatly appreciated. Thanks.

From what I have heard, Aetna is one of the esier ones to get approval through.  No matter which carrier you go through, though, you should have the ability to view the entire coverage policy bulletin before you sign anything.
