Anyone had surgery at a Barix Clinic? Looking for answers.

Hello, I'm new here.  I just started looking into wls even though I'd been to a seminar at a Barix clinic a year or so ago.  I live in Ohio but I'll have to get the surgery done at the Barix clinic in Ypsilanti, Michigan because they take my insurance (Blue Cross Blue Shield Federal).  I was hoping someone could answer some of my questions.  Because it's quite a long drive, I'd like to get as much pre-op stuff done in as few visits as possible.  I go for a consultation on Oct. 5.  I met with my pcp last week and she said she'd write up a letter for the insurance company.  Should I take any paperwork from her to Barix to speed up the process?  How long after a consultation does it take to get actually have surgery?  Also, I have 3 months of dr. history for when I was taking diet pills last year and then I have three months of self-documented diet and exercise after that 'cause I stopped taking the pills and thought I could do it on my own.  Will that qualify for 6 months of diet history?  Any advice anyone could give me would really be appreciated.  I'd like to go to the consultation next Monday with as much paperwork in advance as possible so I do not have to make too many trips to another state.

Yes, I also had Blue Cross Blue Shield so I had to go to Ypsilanti.  They were wonderful and I would highly recommend them to anyone.  You might want to check but I was told the Dr at Groveport Barix now accepts our insurance.  I work for Anthem and plan on checking into this further so that I can transfer down here for my fills since I have the Realize band.
