Can't find transcript of insurance letter!

When I had my surgery someone on here gave me a GREAT letter to send to my insurance company that I just adjusted to my own problems and such. It had great info in it such as how much insurance companies spend on obese patients compared to the cost of the surgery. My friends insurance company denied her and she wanted to send a letter to them along with her appeal. I can't find my letter anywhere. Does anyone have a letter like this or is there one posted on here somewhere? Thanks for any help!
