Diet History HELP

I have my consult with the surgeon on monday. A couple a days ago I received a check list stating that I need a 3 year diet history report. What are they expecting I have tried Weigh****chers, Curves and others but I dont have receipts and stuff. I do have my books and other information. My insurer is BCBS. Can someone help?

I also had no "receipts"  but I had dates of when I belonged to those clubs and how much I lost then how much I gained back.  Gave them to Dr. Ben Mier's staff and they wrote the letter for me.  I have the same insurance. The approval came in two weeks and they scheduled the pre ad testing two days later.  So it will go fast once approved.  Good luck!

Hi, I had to have a 5 year weight and diet history as well.  Luckily I had the same family doctor for many years, and had documents weights almost every year.  My doctor was able to state that I had tried numerous diets without success, and that was enough for my insurance.  Hope this helps Good luck on your journey, and never give up!! :) caroline
