Hi! I chose to got to Miami Valley Hospital in Dayton.  If you hop on 35, it takes you right to downtown Dayton.  I can get from my house to the hospital in about an hour and 25 minutes. They were WONDERFUL!!! I chose to go there because they had surgeons who performed the band, the RNY, the Sleeve, and the Duodenal Switch.  I had found that the surgeons in Columbus only performed the band and RNY.  I got A LOT of BAD information from the seminars I attended.  The only place I found where ALL the surgery types were represented was at premier bariatrics at Miami Valley.  Here's a link if you'd like to see: http://www.premierbariatricassociates.com/ I attended my seminar in October and was approved in November.  I had a surgery date in December but opted to have my surgery after the holidays on January 10. The staff works with their patients in a very caring and professional way.  The follow up care is amazing. The pain management while in the hospital was unparalleled and they really make it a comforable place for the obese patient. Depending on which surgery you want done, some of the insurance you listed will approve before others will.  I know people have a lot of complaints with Care Source.  I would recommend asking the insurance person at your surgeons office (whomever you choose) about recommendations on the insurance issue.  At premier bariatrics, the girl's name is Jennifer.  She handled all my insurance questions and submission and I contribute my fast approval (5 working days) to her. Best of luck!!
