Missing records to document weight

Good Morning All, I have requested my medical records in writing several times to document my weight in 2004 from a former pcp. The ladies in her office are especially nasty and I believe they are dragging their feet because....well, because they can. Is there another alternative to documenting my weight? I can't remember the name of the gyno I saw that year (yes, I've been dr. hopping I've just found a team of dr's I like in this city, and that's important to me). I have my med records from 2000-2006, only 2004 is missing. Could I perhaps submit photos of myself from that year as a testimony that I didn't miraculously lose and gain 100#'s that year?

Have you been requesting or your doctor? If you have, why not try having your doctor fax them. Maybe if they see it's the doctor requesting they might cooperate. You ought to talk to Cora on the Over 50 board, she's having the same problem. I'm requesting records too but so far no problem. Good luck, Peggy

i would go to the Dr office and request the records in person and in writing....get them to sign and date the request..... If they cannot get them for you in a reasonable amount of time I would go back and speak with the office manager. Remember it is the squeeky wheel that gets oiled first

Call you Dr.'s office one more time. Request your records, give them a deadline and make them aware if they don't have them for you, that your attorney will be the next person they talk with. Those are your records and you are entitled to them whenever you want them. I'll bet you get them.

I believe the Health Care Privacy Act HIPPA requires your records to be given to you within a specified period of time. Tell them you plan to file a complaint. They are also supposed to have HIPPA compliance officer at the office, ask them who that is, should get things moving.... mel
