Insurance Coverage....Please Help Me

I was wondering if any one knows anything about Family Health Plus (HMO Medicaid of Care Plus) I am considering having surgery and would like to be on top of my job as to what I need to do. I am going through the Cori Center and was told by my PCC that she is waiting for approval, just want to know would I qualify if I am 31yrs old, I stand at 5' 6" and weighing 247lbs., with no co-morbities, only that I have High Cholestrol and Acid Reflux. If anyone could help me with pertinent information all would be deeply appreciated. Thanks.
