CDPHP Anyone have it?

I am from Albany area and I just switched my insurance to CDPHP (Capital District Physicians Health Plan) I was wondering if anyone has it got approved for a breast lift. It starts in January and all I have left is I want a breast lift and my arms done. I was hoping insurance would cover a breast lift so i thought I would ask around.
