HI I HAVE EMPIRE BC/BS--PPO-AND IM HAVING MY RECONSTRUCTIVE SURGERY ON OCT. 21--THEY APPROVED IT---IF U NEED A DOC IN THE CITY I CAN GIVE U HIS NAME --BUT I DONT KNOW ANY UPSTATE--GOOD LUCK
