Need Sample Letter

I need some help. My PCP is a little wacky and he needs a little help on writing my letter for my. My GF can't find her letter. Is there anyone out there that can tell me were i can get a sample letter? I can use the help. My PCP first attempt on writing me a letter was writing some stuff on a Script pad, the i had to see him yesterday and he want me to get the weight loss clinct to write it and i'v told him 1000 time that Insr. wants him to write it. This is the last thing i need, he did that one on the script pad but i don't think thats going to fly, and i don't want any delays with the Insr. Co. Thxs
