Hi Charlene I have Excellus BCBS. All you have to do is call them, they will answer your questions. They also have a website that answers your questions as well. I will post the site later when I get to work. I don't have the link here at home. When I called them they told me that as long as I meet all the requirements the WLS surgery will be covered 100%. I hope this helps you some. Andrea

Hi Charlene, I have excellus bcbs and they cover if you have a BMI of 40 or greater without comorbid conditions or 35 and above if comorbid conditions exist such as high blood pressure, heart disease, pulmonary hypoventilation, diabetes, sleep apnea, degenerative arthritis of weight-bearing joints or metabolic syndrome. My BMI is over 40 and I was approved within 3 or 4 days. You will also need a psych. evaluation, documentation of of the length and name of previous weight loss programs, they like a list of greater than 3 rigorous attempts. Also a letter of support from your PCP. You shoud call them & request a copy of the corporate medical policy. Also, you should read your policy to make sure that the contract does not exclude coverage. I don't believe they do, but you may have a self funded policy whereas the employer can dictate what is and is not covered. Good luck & keep us posted Tammy

I would look at your policy to see what the coverages are. While Excellus BCBS covers wls--some policies have exclusions. The best bet would be to read and understand your husband's companies policy. Depending on co-morbidities and surgery type there could be coverage or not.
