MEDICAL NECESSITY FORM LETTER

Hi, everyone...........I have been reading your notes and they are helpful and encouraging. I need your help........does anyone have a form letter froma doctor, and can u email it to me? I see my PCP on saturday and would like to give him a copy to write one.............thanks.......

I wrote up a list of everything my pcp should include in my letter & gave it to him. He was great about it & included everything. I was approved by my insurance on the first try. Good luck... Tammy

Hi Tammy, I just wanted to ask you exactly what did you include in your letter for your pcp if you dont mind me asking... I'm trying to get my son approved for this surgery and they want so much now...thanks
