I called and asked if they covered Bariatric Surgery in my plan, once they said yes I asked what do I have to do to get approved. They would only tell me about the letter of medical need but after talking to the my surgeon he knew exactly what they needed to get approval. I would call them and ask them do they cover that and if they do find yourself a good surgeon and let them work on it for you. The office should know what is needed from each Ins Comp. I did not find the Ins Comp gave me much info and not to be helpful in that area, but that is just in my case, others might of have better luck calling the Ins Comp..

Me and Debra have the same insurance, but when I called them they told me everything...I needed, letter of necessity, six month supervised diet...and the testing that the surgeon sent for....
Every insurance company is different but just ask like Deb said if they cover Bariatric surgery and what do you need to be approved....then make an appointment with your surgeon and his office staff will help you with all the requirements....
