Filling out my packet :)

I just got my packet in the mail 2 days ago and I've been filling it out. This is for Dr. Posner/Dr. Forbes in the Buffalo, NY area. I just have to get some medical information from my PCP, submit everything and then hopefully I'm on my way! On the packet it says the schedule is quite full and could take a long time to even get a consultation, which I'll find out about after they review all my paperwork.
