CDPHP... Anyone know how long it takes?

Hi! I'm LeeAnn, kinda new. I just finally decided this surgery was right for me. I saw my PCP this A.M. and will have my referral to Dr. Cole in Corning by Monday morning. My appt with him is 1/11. Still have a ways to go yet. But I was wondering if anyone knew exactly what CDPHP requires and about how long the approval process takes??
