Kimberly-
Great on getting the packet!!! When is your group consult? I suggest scheduling the nutritionist and psych now because they have been booking up quickly. I just had surgery last week with Dr. Boss (O'Malley's partner). I found him to be great and had a great bedside manner as well. His wait was also considerably less. When I had called last April for O'Malley, I could not even get into group consult until 11/18, well I called Dr. Boss and ended up having surgery 11/15.
Good luck with your PCP and the diet history. I know it sucks but they just want proof that you have tried other ways. I was able to make copies of my Weigh****chers books that I had kept for a year and gave those to my PCP. Even though she knew I was doing WW, she had not specifically monitored me while doing so, but that helped her see what I had done.

Hi Kimberly,
My insurance required me to have 6 months of Dr supervised diet too, so when I decided I wanted to do this back in May I immediately started going to my Dr. once per month to get weighed and discuss what I was eating and that I was walking 3 - 4 times per week for 1/2 hour. My seminar was June 1st, initial Surgeoun visit was July 14 and I had all my consults done within 2 weeks of that. Finally all of the consult's were at my surgeons office by the middle of August and I was given a date of September 21. Unfortunately the insurance company denied me because I only had 4 months of Dr. supervised diet/exercise so I went to the Dr in Sept and Oct, faxed those notes to the insurance company and was approved! I had my surgery on 11/11.
So my suggestion to you, even though we have different insurance companies, use your last visit with your pcp as the 1st Dr supervised visit and start going every month to get that taken care of. Ask your doctor for a copy of the notes each time you visit and keep them in a file so you have copies of it if so if someone says well you didnt do this you can say oh yes I did and here it is! I also got copies of all the consult's letters and also the letter my pcp wrote saying that it was medically necessary (containing my diet history) this way I had a copy of everything in case anything got "lost".
Good luck in your journey!
God bless you and keep you.
