Letter of Support from PCP

Does anyone have a good sample letter of support from the PCP?  I just saw the letter that my PCP sent the surgeon for my initial consult but it stinks and is very vague. 

I had requested that they send a letter of support as the ins. person at the surgeon's office said it was necessary and the staff at my PCP's office was going to use that same initial letter.  I told them no that it had to be more detailed but I wanted to provide them with a sample.  Any suggestions where I can find one?

Ask the surgeon's office for one.  Type it up for your particular details/info, then give to PCP - my bet is, he'll just sign it yo get you out of his hair.  LOL

Hello lady!! Ask your surgeon for a sample letter. They gave my dr one and she said I wrote it and Im not doing it again and well her little 5 sentences worked for me.  Insurance companies really hate having to read something long. If it gives the points it needs then it should be okay. Your surgeons office will go threw things and if they feel its not good enough to submit they will tell you. Like I said my doctors letter was like 5 sentences. I was like my god thats not good compared to the letter they told me to give my doctor as a outline of what they wanted.  Talk with your surgeons insurance person they will tell you what you need. Thanks folks!  Wow--that is interesting clad!  I'm going to ask Arnot to fax me a copy of their sample letter. 

I guess I just worry too much about getting approved because my BMI is right at 40 and I have no co-morbidities (right now!!).

I am glad things are going well so far for you! I had my docs appt. yesterday,I am in great health(so far) except for the weight..my bmi is 43..ugh..blood work tuesday and pap after that then he said he would refer me to any surgeon I wanted...Yay! How are thing's going with GHI? I hope all goes smooth..keep me posted!

Hi Kim,

So far so good with GHI.  They didn't pay for my visits with the dietitian or the psych eval but I knew that ahead of time so that cost me about $400.  I have my physical with the surgeon's PA this coming Monday and then final surgeon consult on Aug 7th.  Not sure if they are going to send me for any additional testing but if they don't, I'm thinking the insurance paperwork will be submitted mid-August.  Sometimes I catch myself getting anxious over whether or not I will be approved but I'm trying to keep focused through prayer and positive thoughts!! 

Hope all goes well for you too!  I really think you'll like Dr. O'Malley!  He is one sharp guy!

Five days later, I just received an awesome 2 page revised letter from my PCPs office.  Wow--it was extremely detailed and so thorough!  I couldn't have been any more happier.  I wanted to jump thru that FAX machine and hug that lil girl (ok, ok--so she is 20 something!)

If I'm learning anything about this experience --its that you have to be a true advocate for your health!
