I had to have a referral from my PCP to make the info session appt.  No labs etc.
THe info packet had a form for PCP to fill out with BMI etc.  I attatched my most recent labs but they weren't necessary.

Ask if they have info sessions at any other location.  Mine was in Corning, NY.  I just had to call their office and register for it---they were advertising on the local radio station and I didn't need a referral or anything else to attend.

I am currently in Dr. O'Malley's program.  I have made it through the NUT appts and the next step is meeting with Dr. O'Malley and convincing him that the DS is right for me.  Yes, you will have to make sure you do your research and convince him that you want the DS and why.  When I went for the seminar, I asked alot of questions regarding the DS (he doesn't even mention it in the seminar).  Go to the DS board here, there's alot of information on the DS and alot of people that are very happy to answer any of your questions.  As far as having the surgery before the end of the year, I can pretty much guarantee that it won't be by the end of this year.  I scheduled the seminar in January (my seminar was at the end of April) and you have to go through the NUT seminar and at least 2 NUT appointments before you can even see the surgeon.  That's at least 3 months, just for the NUT.  Right now, appointments to see O'Malley are being scheduled 2 to 3 weeks out and surgery is being scheduled 6 to 8 weeks out from the time you see O'Malley.

Hope this helps.  Feel free to PM me if you have any other questions.
