Dr. Singh in Albany did mine. I had a BMI of like 73. I was @ 500 at surgery. I am down to 340-350...give or take depending on scale calibration and weight of clothes.

Where have you gone for consultation?
What have you found as reply?
Most porgrams require preop wt loss for a variety of reasons....and I must say being able to do a liiquid diet is imperative as we are just about forced to postop for a few weeks....Ellis in Schenectady uses Medifast preop to shrink liver, help those lose wt, adhere to postop diet preop and reduce risks at surgery. Albany Med does preop wt loss 10% by whatever means works for the individual. (I did 1200 ADA calorie diet).

Dr Micheal Gangern is reportidly back in NYC area I would seek out his ractice! He is world renowed for doing first LAPrascopic DS, revisions, high risk cases etc....

Preop dieting is challenging, many find it difficult to embrace the need and to perservere thought the hoops, but those that are able to do this find the benefit so great (getting the life altering surgery) as we learn we will forever be mindful of our diet and lifestyle postop to be successful and need to exercise and manke helathy food choices (I might say perhaps a little less with a duodenal switch which is highly malabosorbtive so one can do more fats..) But sugar can be the enemy of any wls..

For ME, it was "no one" was going to stop me from having this operation, it was my decision and all that was asked of me was to loose 30 lbs, I figured this is a small price for such a wonderful gift I would be given and the opportunity to have a healthier happier longer life....! I was so motivated/psyched; I dropped 30 pounds in 30 days and went on to drop 15 more before surgery! (Although this took me 2 months, as it got harder and I had a few last meals, we all do but you can't let this ruin your opportunity!) I believe it is what allowed me to get to a normal BMI as well! And it is not unheard of for surgeons to postpone or cancel surgeries if one gains weight!!!

I also had to remind myself how BAD I wanted this! Everyday it was/had to be more important to me to change my bad habits and lose weight to have this surgery than to continue to slowly kill myself with food/keep myself from the only hope I had!. I also told everyone about my plans and they helped support me, my friends/family and coworkers! I really can't give you any more guidance than this, it really had to come from within, it was a mindset with me, and I wanted this BAD real bad and I knew it was my last resort/chance! I felt invigorated and motivated by the end results...the long-term had to outweigh the short-term gratification of eating bad choices or overeating! It was a learning opportunity for me to change my habits and practice chewing well, not drinking with meals and eating smaller portions, giving up sugar/caffeine/carbonation/alcohol (if any of those are your issues, carbonation/caffeine/alcohol weren't issues for me)!

So a few weeks/months of healthy habits to lose pre-op wt is not a lot if you frame it right! IT IS NOT FOREVER and it is about starting this journey!

Of course my BMI was 47, and i was 320 preop not exactly where you are at, I have encountered many similar to yourself on my journey many who didnt like the preop requirement for whom they needed to sometimes loose 100# before surgery but so many did it and how amazingly it helped! Hope you keep posting, thre is much streength, hope and support here!

Not sure what type of surgery you are looking for but I hope you research the BPD/DS as well for those with HIGHER BMI I think it is maybe the best choice to get one to lowest BMI possible postop!

My surgeon, Dr. Dominick Artuso, is in Dobbs Ferry.  He will do surgery on high BMIs.  Good luck!

Okay, so you not only need a surgeon who does higher BMIs, more importantly you need a special surgery who does revisions (you have to go to someone *****ally knows what the heck they are doing).  I read that you had a LapBand removed - we build much scar tissue from any surgeries.  It is a tedious disection task once they get in your abdominam.

Think about getting the biggest and best gun - the Vertical Sleeve Gastrectomy with Duodenal Switch or simply, the DS.

I love my doc, he does ALL of  the WL surgeries.  He is Mitchell Roslin, M.D. w. Lenox Hill Hospital on the Upper East Side of Manhattan.

There are other great docs  in NYCity.

Good Luck

Thank you so much!  I have an appt with Dr. Herron at MT Sinai, and Dr Brackman in Melville, but I am going to call Dr Roslin, too - Dr. Herron recommended the DS - you sure you're not a doctor? :)  I appreciate the feedback, I need all the help I can get!
