Just wondering who I can go see to have this done??  Psychologist, Social worker, therapist??

If you want to get somewhere you have to know where you want to go and how to get there. Then never, never, never, give up!

I saw a psychologist or maybe he was a psychiatrist.  I was given a list of acceptable people to perform the psych eval and what insurance they accepted.  I would think that your surgeon should be able to provide you with the name of someone who can do this eval.

Good Luck

This may depend on surgeon and or insurance company!

For instance I am a psychiatric Nurse Practitioner, and I have doen hundreds, some programs state psychologists only..best to ask! Of course IMHO the best person to go to is if you currently see a behavioral health provider! They know you best and could do the assessment! Ask for recommendations, many folks have no clue what is being asked for....

Hi there,  I called my insurance company and they gave me some names and a website to look up more Doc on your insurance.  I used a Psychologist in Yonkers, NY

I am just waiting on the approval letter, Tentitive date scheduled April 9, 2009. This is being done for medical reasons and hoping it turns out even better than planned.

Good Luck
