I have Dr Sadasaviam in the same office, I requested Dr Moon but they gave me him instead. I heard Dr Moon is great, Dr Sad seems very nice too. The steps to get a surgery date are as follows: 1) You have your initial consult with Dr Moon, he goes over everything with you. 2) They set you up with the group consultation, you go into a room and sit with about a dozen other people and see the NUT (nutritionist), SS worker and then the insurance lady, they will give you information on what medicaid will want from you and such. (about 3-4 hours long) 3) the same time they set you up for the group consult they will set you up with 2- 1hour meetings, 1hr with the Nut and 1 hr with the SS worker they just ask you questions about yourself and why you are looking to have WLS.
4) if the Nut and SS are satified with your answers, they will give you the ok to set up a date for your physical **if you have co-mobesity problems, they will want them addressed also (sleep apnea, any heart problems or lung problems) you will have to have clearance for them too. 5) once you are cleared from your physical, then you see the dr again (approx 2 weeks). after you see him, then he submits all the information to the ins company for approval. then you sit and wait till you get an answer, (you would have to ask medicaid how long they take to approve) 6) if your approved, then they schedule you for a surgery date, I was approved on april 21st, and my surgery date is July 1st. that was the first opening.

I was just on your profile. In the 4 hour session you had they gave you a sample letter to give to the dr. one of the other pieces of paper they gave you was your diet history. fill that out, and make a copy of it. and give your PCP the sample letter and the diet history. Tell him to use that letter as a reference for what he needs to write. Who is your dr? (just curious) if your blood pressure is spiking you'll have to have that treated. get it done now. or it will just hold you back.
