Application/paper work help

HI! I finaly recieved my applicaion in the mail from Dr. Posner office- I was unable to priont DR Caurana application because my printer is not working- SO I emailed them and asked them to mail it to me- DId any of you have problems filling this out? Their are so many questions that I am not sure of the answers- Such as the dates of my diets and how much I lost and how much i gained back-- I Serously cant not remember-  thanks

Did your PCP weigh you? Do you have old WW books or things? A journal? If it is imperative you remember call on all the resources you can! Jamie

Just do the best you can...I had been dieting for 40 years so it was a process to list all my attempts at losing weight.  The office crew at Dr. Caurana's was very helpful in the paperwork to be submitted to the insurance.  Congratulations on taking the first step...keep us updated. Rosemary

you just do the best job you can. i try to think of things that were going on in my life and that helped me out. i am going through dr. posner's office also, the application is daunting
