hello anybody in long island new york have blue cross blueshield insurance what are the requirements for approval?

Not me (though I MAY still be able to help), but I have a suggestion: you didn't mention WHICH BCBS you have... there are many, and all have their own guidelines (though some may be exactly the same, most have at least subtle differences in things like which procedures they cover, length of supervised diet if any, etc, etc) Good luck.

Sean is so right!  Each BXBS has different guidelines.  Once my BCBS approved the procedure, they expected a 5% wgt loss in order to approve.  That 5% began with my first appt. to my surgeon's office.

You might even be able to bring your insurance guidelines up on your computer and print it out. You must have access to their website. That's what I did and read it with a fine tooth comb and then spoke to customer service to verify. Good Luck.

I have BCBS and it's a PPO. Find out which one you have.   I called my plan when I started and they gave me all of the details over the phone.   They took my information, assigned me a case manager who is a RN.  She reviewed everthing and I got approval in 1 day.  I've heard may stories about the insurance companies .. I guess I was lucky.   I Stick with it, I wish you the best of luck ..

thank you when I called one person at BCBS they said its not covered, then another said it was, they said a doctor has to write a strong letter saying I need surgery Im 290 and 5 feet 7 (i think thats reason enough!)so I hope they do not given me any problems ,I just started going for my pre opt tests I have to have a stress test Yikes!!!!!!!!!!!!!
