dietician Long Island or NYC

Just wondering if anyone here is aware of a dietician who works with bariatric patients located in the Long Island or NYC area who will work with another doctors patient? I'm am going on 5 years post op and am looking for someone who will help me with my diet. I've called a few places and left messages and no one has returned my call. I didn't realize what a stigma it is to see another doctor patient. If anyone one has any suggestions please let me know.
