Surgeon That Accepts Medicare...
I can't say that they take high risk patients, but my suggestion is to seek a surgeon who does revisions and/or ones who've handled other possibly high risk patients.. The reason? Doc's who are willing to do revisions are more apt to take a high risk patient - revisions are often considered higher risk than standard WLS. So may I suggest the following, who appear to have accepted Medicare in recent months:
Dr. Cottam - one of his patients I personally would class as high risk, having had tumors and diabetis, and 3 brain surgeries. You can read his profile here - http://www.obesityhelp.com/morbidobesity/bariatric+surgeon+profile+Daniel+Cottam+MD+qqm.ht

Dr. Ganser shows to have an older patient (60+) with a high BMI (60+) on medicare http://www.obesityhelp.com/morbidobesity/bariatric+surgeon+profile+John+Ganser+MD+fpl.html
If that doesn't work for you, let me know and I hope you'll let me know how it DOES work for you too
Best of luck!
