I was from Berwick also! I left the end of March/09 for Calgary AB...Here is the referral you will need to give to your doctor.

First of all Jackie there is a bit of research you need to do first. Once you have selected which surgery you want. Pending where the procedure will be done you may need a doc's referral.
If you get selected for the VGS in Halifax ypu will need to go through a few hoops. If you go to NB to have your surgery you will need a referral from your Doc. Their waiting lists are very long. If you choose to go out of province you will have the freedom to have it when you want to. But you need to know that out of province is not covered by MSI.
Even if you go to New Brunswick you have to pay for the band but MSI will cover the hosp and office visits. You have to apply for it first. That should give you some assistance to get you started.
