list of questions to ask

ok so i've been giving out the wrong profile name -- it's not Dr X it's Dx E -- i had the D and X right -- here is the list of questions to ask -- personally i think this is a wonderful list :) and thanks to Dx E for listing it on his profile  and i just looked and it's not there anymore :(  now what to do..............
