BCBS (ppo) requirements..any help please

I spoke with my provider and they said that it is covered as long as I have met there requirements. Anybody else had this...and what were they.

Hello, The only way you are really going to know what their requirements are is if you call them. Many people may have the same insurance company but depending on who your employer is and the kind of coverage they bought requirements could differ. Also you should be able to check on line with your specific insurance policy. I would just give them a call tell them your Primary Care Doctor is suggesting this procedure and you would like information on what the requirements are to get approval for surgery. Good Luck. I have Anthem PPO. With my BMI and osteoarthritis I met the requirements that my plan has. They paid 100% of my surgery. If you call the phone number on your card, they'll tell you what you need to do to meet their requirements. Feel free to email me if I can help. K

Lesli, I had BCBS, and the only requirement was a nutrition consult and psych evaluation. It was approved immediately, no questions asked. Good Luck!! Carrie
