Has anyone written an appeal letter?

I have to write an appeal for my isurance company. Not sure how to go about this. Has anyone already done this, can you send me a template please. Right now i am just a little concern and not sure how to do the appeal because i'm not appealing that they turned down my surgery saying i don't need it, i'm apealing because they now say my insurance doesn't offer it. So can anybody help me write an appeals letter. Someone must have been denied for one reason or another. Thanks

HI - I was in a similar position earlier this year with a policy via my husband's work that excluded WLS. I wrote my letter to the head of HR. for the corporation.  Her response was initially pretty dismal and said that "it is what it is."Not only did I send my letter, but I also sent copies of the HPHC, Anthem, Medicare...coverage benefits. In the mean time, I decided to buy my own Insurance policy (HMO Blue NE). They covered WLS with no questions asked. ( I had surgery on Nov 5th! YEAH!!!!) Surprisingly, I just got an e-mail this week from the HR lady stating that my personal plea and information had prompted the company to review their coverage. They retroactively are adding Gastric Bypass to their coverage effective 10/1/07.!!! She sent a very nice note that my letter had made a difference. I was happy to help those who come after me...but bummed that I have been paying $600 a month for coverage- that now I find I didn't need. Oh well, everything happens for a reason. If I have helped others- then I am happy. Plus I have had my surgery!!!!! So......the moral of the story is- do not get discouraged. I would be happy to send you a copy of what I wrote- you could modify it. Feel free to send me a way to contact you. Keep your chin up!! Carolyn

I was told today by an insurance broker that i wouldn't be able to get coverage in NH because i am overweight. I pay $412 a month now for cobra which i was told by my insurance that the surgery wasn't part of my plan after i received and email that it was way back in Feb. So when i got layed off i kept my cobra because of this. And now there saying sorry we made a mistake, its not covered. So i went to the insuranc broker and she said i wouldn't be able to get it, so i'm thinking in January that i would find insurance that covered this surgery, i think i am more interested in doing what you did. Can you give me more information on that? Where did you go to get your insurance and when you were writing up your plan what did you chose, who did you talk to? Please eamil me ASAP so i can probably drop the cobra and buy my own insurance to pay for it. Any downfalls on how long you have to keep the insurance for? The good news you can claim it on your taxes as a write off. I would also love a copy of your letter, that i can send to the HR person where i was working so maybe they can include it for January 1st when there looking at other insurances. thanks so much. Which surgery did you have? I'm getting the lap-band, and hopefully they will cover the fills too.
