I went to the class last night but can't seem to find the card that had the fax # on it so I could send my doc's referral form to them. Ask-A-Nurse didn't have it and when I called the center, I got voicemail. UH. I'm frustrated and I want to get the ball rolling.

If you go to the webstite you can get the information you need .catholicmedicalcenter.org, look for the Obesity Treatment Center, if you can't find it there, I would either cal the hospital or information to get the number. Good lUck!
