Wowza, killer questionnaire

I just received the questionnaire from Dr Hung's office. It was alot more than I was expecting. Who knew I'd have to keep track of all the diets I've tried over the last 10 years? Is it bad if I don't have documentation? I also didn't do all my diets under Dr supervision, like the Atkins diet I just did on my own. I'm worried about that. Anything else on the questionnaire I should be paying close attention to? I don't want to answer a question wrong and have it ruin my chances for WLS later.

These questions will help the Drs. office get you approved by your insurance. Even tho you don't have documentation of the diets you were on list them anyway!!! Write down what happened, how much you lost or didn't lose and how long you kept it off. If you have been in any group type weight loss programs and have any of the booklets or can get verification from them ( like weigh****chers) get whatever you can. I even put my goal weight pin from weigh****chers in my scanner and printed a copy of it for my dr. Your health history (co-morbidities) Any problems you have health wise that are effected by your weight is also very important. Sleep apnea...diabetes.. heart problems.. leg swelling.. asthma .. high blood pressure .. knee or feet problems or pain .. back problems anything that is either caused by or made worse by your weight. If you have been to the dr for any of these things get your medical records from your PC and have it sent to Dr. Hungs office. Every time you go to your family dr. don't they weigh you ?? that is a documentation that shows any ups and downs in your weight. Have you ever joined a gym to excercise?? Ask them to write something out to show you were a member at one time. Were you ever on a baseball team or basketball team get someone to write it out showing you participated. I also had my chiropractor write something out saying my knees and back were effected by my weight. The more you provide the more amunion Dr Hung has for your insurance company.
