HELP... Anyone Approved w/UniCare PPO Midlands Choice?

Has anyone been approved with UniCare PPO Midlands Choice? I have PCOS, Hypertension, Infertility, Ankle, Knee, and Back pains. My family history is not good. My father had Diabetes and congestive heart failure and died at 49. Other family members suffer from Diabetes, Heart Failure, Cancer, Obesity and PCOS. My husband is very supportive in my journey to have WLS, but he is not sure if I will get approved. Thanks, Stacey W.
