Need Some Help With paper work

I'm filling out paper work for the Dr & im confused on 2 of the questions, this is how its stated Disabled-reason for disability ________________ Onset of disability ____________________ I 'm just confused on what them mean for both Thanks Janelle

My interpretation would be if you are disabled why are you disabled what caused it ? ( for the first question) the date or approxomate time that your disablity started ( for second question) ...
