I'm including the link to the NC Medicaid Guidelines for Bariatric Surgery so that you can see the details, but here is the high level overview:
* You need to have a BMI or Body Mass Index of 40 or more without comorbidities or 35 or more with one or more comorbidities
* You need medical documentation showing that your weight has been in the over 40 or 35 BMI range during the past two years. If you have been to the ER once a year in the past two years and they took a height and weight, that may satisfy the requirement.
* You need medical documentation that you have participated in a medically supervised weight loss and exercise program for 6 consecutive months within two years prior to approval. (You don't necessarily need to have been successful, but you have to have participated and tried.) Your progress regarding your dietary and exercise regimen needs to be entered into your medical records once a month by your Dr. This can be your primary care Dr or your WLS Surgeon.
Or You can participate in a surgical prep program that has a 3 month diet program, with a behavior modification program and an exercise program that your Dr. documents in your medical record... Again, you have to show that you are trying.
* You have to be evaluated to be sure that you are emotionally stable enough to have the surgery.
It also addresses the types of weight loss surgery the Medicaid will cover, whi*****ludes the RNY, LapBand, Biliopancreatic Diversion with or without Duodenal Switch, and Vertical Gastric Banding.
